Job Summary
Conduct psycho-social assessments and coordinate discharge planning for high-risk patients to optimize throughput and reduce readmissions. Complete thorough chart reviews, attend daily bed huddles and care conferences, and collaborate with interdisciplinary teams to ensure timely transitions to post-acute care. Proactively educate patients and families on advance directives, community resources, and post-hospitalization options while advocating for scarce services. Maintain active communication with physicians and post-acute providers to develop collaborative treatment strategies and secure appropriate facility placements. Document all interventions and activities in designated software in compliance with HIPAA guidelines. This role requires a Master's in Social Work and state licensure, with 2nd and 3rd shift exclusive scheduling.
Required Qualifications
- Graduate of an accredited masters' school of social work
- Licensed as an SW to practice social work in the state where work is being performed
- Meets contact hour requirements for licensure, including all state required courses
- Knowledge of psychosocial issues re: health related problems and post-acute care needs
- Excellent communication skills and the ability to coordinate care well with an interdisciplinary team
- This position works 2nd and 3rd shifts exclusively
