Job Summary
Plan social services programs for adult and geriatric residents, addressing therapeutic, neurological, dementia, dialysis, IV therapy, and infectious disease needs. Demonstrate knowledge of age-specific developmental factors to tailor services effectively. Promptly follow up on admission inquiries, evaluate prospective residents, and assess their needs to determine appropriate facility placement. Requires at least one year of social work experience in a large long-term care setting, with a BSW or MSW preferred. Health, dental, and vision insurance, plus 401K and tuition reimbursement are available.

- Experience as a Social Work in a large LTC setting preferred
- BSW or MSW preferred, or Bachelors degree in related field
