Job Summary
Assess client needs through interviews and case history to determine case management requirements and therapeutic approaches. Coordinate support services, administer program curriculum, and transport clients to appointments and service locations. Provide customer service and advocacy while documenting service delivery, progress, and case management processes accurately. Process new clients, issue clothing, and prepare case files for periodic review and quality assurance across residential, community, and jail-based settings. Maintain current knowledge in community health, harm reduction, and behavioral health services through training and networking.
Required Qualifications
- High School Diploma or GED
- Two (2) years of work experience involving substance abuse, healthcare, law enforcement, emergency management, or corrections
- Possession of a valid New Mexico driver's license by the date of hire and maintenance of a valid license while employed in this position
- Successful completion of a post-offer employment medical examination and background investigation
- Adherence to all County safety guidelines
- Complete all FEMA training(s) assigned to this position
- Complete required supervisor training, if applicable
- Maintain confidentiality of sensitive information
