Job Summary
Conduct integrated psychosocial assessments and develop intervention plans; provide brief counseling toward care plans and transition services including Infant Case Management; coordinate with the interdisciplinary Maternity Support Services team to address client needs; perform systematic social services screenings, assist clients with access to services and self-referral, and advocate on behalf of clients; document services in the electronic health records, maintain clear records, and participate in interdisciplinary team meetings to inform care planning; deliver culturally sensitive, crisis intervention, problem-solving, and coping skills support; educate clients on personal and family health/mental health care options and coordinate timely referrals to behavioral health and community resources; employ AIDET skills in all client interactions and build strong relationships within the service system to support pregnant and parenting clients; demonstrate knowledge of growth and development across the lifespan and adapt care to age-specific needs; maintain professional standards in accordance with agency policies and applicable regulations.
Required Qualifications
- Bachelor’s degree from an accredited university
- Current Certified Counselor through the State of Washington Department of Health or Chemical Dependency Counselor
- Washington State driver’s license
- Background check from Washington State Patrol
- TB test annually
- Basic Life Support (BLS) CPR within 90 days of hire and maintained
- Pre-hire and annual employee health screening
- Annual influenza vaccine or approved exemption
- Ability to work with underserved populations
- Experience coordinating referrals to behavioral health services
- Experience with crisis intervention
- Knowledge of patient-centered health home model of care
- Experience using electronic health records (EHR) and Microsoft Office
Additional Requirements
- No history or evidence of alcohol or other drug misuse for 3 years prior to employment
- No felony conviction within the last 7 years or any history of child abuse/violent crime
