Social Services Specialist II (76853)

Conduct psychosocial assessments and create individualized service plans for clients while ensuring expedient access to resources such as food, housing, and healthcare. Engage in face-to-face contact for approximately four to five hours daily, manage assigned caseloads using evidence-based interventions, and submit thorough documentation within required deadlines. Collaborate with interdisciplinary teams, healthcare providers, and case managers through regular huddles and meetings to coordinate care and maintain high-quality service standards. Maintain professional relationships with community partners and participate in ongoing coaching and continuing education to support client self-management and treatment adherence.
