Job Summary
Social Services Personnel at Samaritan Nursing and Rehab are responsible for planning, developing, organizing, implementing, evaluating, and directing social service programs; attend meetings with administration, medical and nursing staff; assist in policy administration and coordinating department procedures; participate in community planning related to residents and families; participate in discharge planning and resident assessments; refer residents/families to appropriate social service agencies; maintain quality relationships with medical professionals and health-related facilities; join the team to make a real difference.
