Job Summary
Conduct initial patient assessments and document visits, contacts, and care plans on patient charts. Serve as a resource liaison by placing phone calls and providing community resources and placement options to patients and families. Deliver clinical counseling and support while attending weekly team conferences. Maintain a valid driver's license and hold a Master's degree in social work with three years of agency or hospital experience. Work in home health and hospice agencies across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
