Job Summary
Conduct initial patient assessments, document visits and contacts on charts, and place phone calls as needed. Provide community resources, placement options, and counseling to patients and families. Serve as a resource liaison and attend weekly team conferences. Maintain a valid driver's license and complete required vaccinations. Located in Riverbank, CA, with a Master's in social work and LCSW preferred. Adventist Health offers integrated care across 440 sites, including home health and hospice agencies.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
