Job Summary
Conduct initial patient assessments, document visits and contacts on charts, and serve as a resource liaison by placing calls and providing community resources to patients and families. Deliver counseling and placement options while attending weekly team conferences. Maintain a valid driver's license and hold an LCSW license where required. Located in San Andreas, CA, this role supports Adventist Health's home health and hospice agencies across the West Coast and Hawaii.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
