Job Summary
Conduct initial patient assessments, document visits and contacts in patient charts, and serve as a resource liaison by placing phone calls and providing community resources to patients and families. Deliver counseling and placement options while attending weekly team conferences. Requires a Master's Degree in social work, an LCSW license, and three years of experience in a health or welfare organization. Valid driver's license and age 21 or older are mandatory. Located in Jamestown, CA.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
