Job Summary
Conduct initial patient assessments, document visits and contacts, and serve as a resource liaison for patients and families in their homes. Provide community resources, placement options, and counseling while attending weekly team conferences. Maintain a valid driver's license and complete required vaccinations as part of Adventist Health's safety protocols. This role supports home health and hospice agencies across California, Hawaii, Oregon, and Washington, offering personalized in-home care to the entire community.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
