Job Summary
Conduct initial patient assessments, document visits and contacts on charts, and serve as a resource liaison by placing phone calls and providing community resources to patients and families. Deliver counseling and placement options while attending weekly team conferences as a competent clinician. Requires a Master's in social work, LCSW license, and valid driver's license. Located in Groveland, CA, with Adventist Health's integrated health system serving West Coast and Hawaii communities.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- Receive all required vaccinations as a condition of employment
- Acknowledge that you have accessed and read the E-Verify Participation and Right to Work notices and understand the contents therein
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
