Job Summary
Conduct initial patient assessments, document visits and contacts on charts, and serve as a resource liaison by placing phone calls and connecting families with community resources. Provide counseling and placement options to patients and families while attending weekly team conferences. Maintain a valid driver's license and hold an LCSW license, with a Master's in social work and three years of experience in a health or welfare setting. Work across home health and hospice agencies throughout California, Hawaii, Oregon, and Washington.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
