Social Services Practitioner (MSW) (Home Health)

Job Summary
Conduct initial patient assessments and document visits, contacts, and care plans on patient charts. Provide counseling, community resources, and placement options to patients and families while acting as a resource liaison. Attend weekly team conferences and perform other assigned clinical duties as needed. Requires a Master's Degree in Social Work, three years of experience in a health or welfare setting, and a valid driver's license. Licensed Clinical Social Worker status is preferred. Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
