Social Services Practitioner (MSW) (Home Health)

Conduct initial patient assessments, document visits and contacts on charts, and serve as a resource liaison by placing phone calls and connecting families with community resources. Provide counseling and placement options to patients and families while attending weekly team conferences. Maintain a valid driver's license and hold an LCSW license as required for clinical care delivery.
