Job Summary
Conduct initial patient assessments, document visits and contacts, and serve as a resource liaison for community resources and placement options. Provide counseling to patients and families while attending weekly team conferences. Maintain a valid driver's license and complete required vaccinations as part of the home health role. Located in Waterford, CA, this position requires a Master's in Social Work and an LCSW license.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree in social work or equivalent combination of education/experience
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- E-Verify participation acknowledgment
Desired Qualifications
- Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW) in state of practice
