Social Services Manager

Assess psychosocial status of patients, communicate findings to registered nurses, and carry out social evaluations to plan person-centered interventions. Maintain clinical documentation, coordinate referral services for residents and families regarding practical and environmental needs, and serve as a liaison between patients and community agencies. Participate in total plan of care development, case conferences, and discharge planning while assisting the physician in understanding social and emotional factors affecting health. Establish behavior monitoring and interventions to reduce medication necessity and prevent harm. Requires a Bachelor's or Master's in social work with an LCSW, LSW, or MSW license, a valid driver's license, and preferred post-acute care experience. Based at a 34-bed short-term post-acute facility with mandatory background checks prior to patient contact.
