Social Services Manager

Assess psychosocial status of patients and communicate findings to registered nurses. Carry out social evaluations to plan person-centered interventions and maintain clinical documentation for referred patients. Coordinate referral services and community agency connections for residents, families, and caregivers while serving as a liaison. Participate in total plan of care development, case conferences, and discharge planning. Establish behavior monitoring and interventions to reduce medication necessity and address resident behaviors preventing harm. Maintain collaborative relationships with facility co-workers and physicians to support care. Requires Bachelor's or Master's in social work with LCSW, LSW, or MSW license. Valid driver's license required. Post-acute/skilled nursing experience preferred.
