Job Summary
Directs, plans, and organizes the Social Services Department, implementing social care plans and resident assessments per federal, state, and local standards. Develops written plans of care for each resident, reviews assessments quarterly, and interprets department policies for employees, residents, and agencies. Participates in discharge planning, interviews families for social history, and provides information on Medicare/Medicaid and financial assistance programs. Reviews and revises care plans based on survey inspection deficiencies, ensuring documentation accurately reflects services and resident responses. Assists in recruiting, screening, hiring, training, and coaching a team of qualified individuals while adhering to safety regulations and resident rights. Extensive travel including overnight stays as necessary.
Required Qualifications
- Bachelor's degree in social work
- LSW certification
- Must meet State requirements nad regulatory code(s) for licensure certification
- Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment
- Exemplary computer skills that include the knowledge of the Microsoft Suite of products
- Extensive travel including overnight stay as necessary
