Job Summary
Social Services Director role delivering medically related social services to achieve the highest level of physical, mental, and psychosocial well-being. Responsibilities include applying clinical and human services knowledge (social work principles, psychosocial assessments, interventions), managing case management, discharge planning, and crisis intervention; fostering empathetic, effective communication with residents, families, and interdisciplinary teams; creating a comforting, engaging environment; promoting collaboration across departments and organizations including regulatory agencies; and upholding C.A.R.E. values (Compassion, Accountability, Relationships, Excellence) to support seniors in Indiana. The position emphasizes leadership, teamwork, and a commitment to senior care excellence, with opportunities for growth and development within American Senior Communities.
Required Qualifications
- Bachelor’s degree in social work or in a human services field required
- Strong passion for geriatric advocacy and commitment to senior care excellence
- Previous social services experience preferred
- Demonstrates C.A.R.E. values to residents, family members, customers and staff
