Job Summary
Social Services Director – Social Worker position at The Plaza at Lubbock (Lubbock, TX). Responsible for coordinating admissions and discharges, developing discharge plans with involvement from family and community services, addressing resident and family grievances, and acting as a resident advocate when needed. Requires a Bachelor's Degree in social work, LBSW or LMSW, Texas Social Worker license, and at least one year of supervised healthcare experience (skilled nursing facility experience preferred). The role sits on the interdisciplinary team and supports care planning and coordination of referrals, with a focus on resident welfare and regulatory compliance. Required Qualifications
- Bachelor’s Degree in social work
- LBSW or LMSW
- Current Social Worker License from the State of Texas
- At least one year of supervised experience in a healthcare setting working directly with clients/residents
- Skilled nursing facility experience preferred
- Ability to plan, organize and implement social service programs for residents with varying needs
- Current knowledge of state and federal laws applicable to social work in long-term care
- Excellent interpersonal and communication skills
