Job Summary
Social Services Director overseeing resident social care, assessments, discharge planning, and plan-of-care development within Stroudsburg Post Acute Nursing and Rehabilitation. Responsibilities include creating written care plans, coordinating MDS-related components, reviewing nursing notes to adjust care plans, and delivering exceptional customer service to residents, families, and staff. Requires a Bachelor's degree in Social Work or related field with at least 2 years in social work, preferably in healthcare, and strong communication, assessment, and planning skills.
