Social Services Director

Compile social history data and participate in assessments to develop and evolve resident care plans. Lead weekly interdisciplinary care planning meetings and document timely progress notes adhering to state and federal regulations. Coordinate discharge planning for residents and interpret established social service policies while managing community agency services for continued care delivery. This role requires a B.A. in Social Work with a current license, an MSW or LCSW preferred, and at least one year of skilled nursing facility experience. The position is full-time based in Gloucester, MA.
