Job Summary
Oversee the facility's social services program in accordance with federal, state, and facility regulations. Conduct psychosocial assessments for residents upon admission and throughout their stay. Develop and implement individualized care plans addressing emotional and social needs. Provide counseling and support to residents and families during transitions and challenging situations. Coordinate discharge planning to ensure safe transitions to home or other care settings. Assist residents and families with accessing community resources and support services. Participate in care plan meetings and collaborate with nursing, therapy, and administrative staff. Advocate for resident rights and help resolve concerns or grievances. Maintain accurate and timely documentation in compliance with regulatory standards. Provide guidance and education to staff regarding residents' psychosocial needs.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field
Desired Qualifications
- Prior experience in long-term care or skilled nursing facilities
- Knowledge of CMS regulations, care planning, and discharge planning processes
- Strong communication, problem-solving, and interpersonal skills
- Ability to build relationships with residents, families, and interdisciplinary team members
