Job Summary
Social Services Director at Pinnacle Point Wellness & Rehabilitation leads the facility's social services program for residents, coordinating assessments, psychosocial and emotional care, discharge planning, advocacy, and compliance with NASW ethics and regulatory requirements. Responsibilities include conducting comprehensive social histories and psychosocial assessments, developing individualized care plans, providing counseling and support to residents and families, coordinating referrals to community resources, managing discharge transitions, ensuring resident rights, and collaborating with nursing, therapy, administration, physicians, and families. Requires a Bachelor's in Social Work or related field, Missouri licensure/certification as required, and at least 1 year of supervised geriatric social work experience; preferred familiarity with MDS, Medicare/Medicaid, and state survey processes. Strong communication, documentation, and organizational skills, with a compassionate, resident-centered approach to long-term care.
Required Qualifications
- Licensed, certified, or registered in the State of Missouri as required by professional practice standards
- Bachelor's Degree in Social Work or Human Services field (Social Work, Sociology, Psychology, Rehabilitation Counseling, Special Education or related Human Services discipline)
- Minimum one (1) year of supervised social work experience in a healthcare setting serving geriatric populations
- Strong knowledge of resident rights, care planning, discharge planning, and long-term care regulations
