Job Summary
Direct, plan, implement, evaluate and oversee the facility’s Social Services Department to meet medically related emotional and social needs of residents in accordance with applicable standards and policies. Responsible for reviewing policies with the interdisciplinary team, developing social work policies, ensuring government documentation compliance, documenting psychosocial progress, promoting resident rights and psychosocial well-being, coordinating interdisciplinary care plans, providing therapeutic interventions, educating residents, families, and staff, coordinating discharge planning and referrals to appropriate services, and supervising students assigned to social services.
Required Qualifications
- Bachelor's Degree in Social Services required
- A minimum of two years experience in social services
- Long term care experience is preferred
