Job Summary
The Social Services Director is responsible for identifying and addressing psychosocial, mental, and emotional needs of residents. This role involves conducting psychosocial assessments, maintaining compliance with documentation regulations, supporting residents' self-determination, and developing interdisciplinary care plans. The Director coordinates discharge planning, educates residents and families about facility services, and collaborates with the nursing team to uphold resident rights and psychological well-being. Candidates must hold a Bachelor's Degree in Social Work or related field, relevant licensure, and one year of social work experience in healthcare.
Required Qualifications
- Certified, licensed, or registered in the state of practice, required.
- Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
Desired Qualifications
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
