Social Services Director

Review and set policies for resident care, quality assurance, and social work documentation. Meet government requirements, document psychosocial needs, and prevent resident abuse. Work with interdisciplinary teams to protect resident rights, provide therapeutic interventions for transitions, and coordinate discharge planning with home care referrals. Complete social histories and assessments for every resident to address emotional and psychological needs. This role supports the overall operation of the facility's Social Services Department at Onyx of Bernice, a long-term care facility.
