Job Summary
Coordinate and direct the Social Service/Discharge Planning Department to integrate activities within the hospital system and ensure center policy is followed. Perform psychosocial assessments on all residents, develop measurable plans to enhance daily social services, and formulate teaching plans based on identified learning needs. Provide individual and group support to residents and families during adjustment, crisis, or need while maintaining confidentiality and interpreting residents' rights. Interact professionally with medical staff, departments, and employees to maintain a culture of safety and quality. Manage equipment safely, react decisively in emergency situations, and support clinical staff with appropriate resources. Maintain a good rapport with all levels of the organization while treating patients and families with respect and dignity.
Required Qualifications
- A high school diploma/GED
- current State Clinical Social Work Licensure
- Experience in social work
- Experience in long term care environment
- Ability to read, write and speak the English language
- Ability to work cooperatively as a member of the team
- Ability to communicate effectively with residents and their family members, and at all levels of the organization
- Ability to react decisively and quickly in emergency situations
- Ability to maintain confidentiality
- Knowledge of training techniques for clinical staff
- Good ethical practice
- Relationship management
