Job Summary
Assess patient needs, develop and implement care plans, and coordinate resources to ensure comprehensive support. Work closely with healthcare professionals and community organizations to advocate for patients and address social, emotional, or economic challenges. Conduct daily meetings to review cases, perform patient interviews, and lead training sessions on social services best practices. Engage in community outreach to strengthen partnerships. Monday through Friday schedule with a focus on balancing caseloads and timely follow-ups.
