Social Services Director (LSW)

Job Summary
Complete comprehensive social histories, psychosocial assessments, and resident evaluations to identify social, emotional, and psychological needs. Participate in the Minimum Data Set (MDS) process to guide individualized care plans and develop, implement, and monitor social service interventions. Provide emotional support, counseling, and therapeutic interventions while advocating for resident rights, self-determination, and informed decision-making. Coordinate discharge planning with referrals to home care and community resources. Collaborate with interdisciplinary teams to address psychosocial needs, promote resident quality of life, and prepare for state and federal surveys. Maintain accurate documentation of assessments, progress notes, care plans, and PASARR records in compliance with CMS regulations and the NASW Code of Ethics.
Required Qualifications
- Current licensed, certified, or registered Social Worker credential in the state of practice
- Bachelor's Degree in Social Work (BSW) or a related Human Services field from an accredited college or university
- Sociology
- Psychology
- Rehabilitation Counseling
- Special Education
- Related Human Services disciplines
Desired Qualifications
- Minimum of one (1) year of supervised social work experience in a healthcare setting working directly with geriatric populations
- Experience working in a skilled nursing facility (SNF), long-term care, nursing home, rehabilitation center, or senior living environment
