Job Summary
Oversee the development, implementation, supervision and ongoing evaluation of the Social Services Department designed to meet and assist residents in attaining or maintaining their highest practicable well-being. Ensure medically related social services are provided in accordance with State and Federal regulations; complete/direct the social services component of the comprehensive assessment; contribute to social services goals and care plan; supervise preadmission services; facilitate residents’ safe transition back into the community via discharge planning and community-based follow-up care; establish departmental QA procedures and modify them as needed. Requires a bachelors degree in social work or related human services field, one year of supervised social work experience in health care, and a Social Worker license in the applicable state; offers competitive salary and professional development opportunities.
Required Qualifications
- Minimum of a bachelor42s degree in social work or another human services field to include sociology, gerontology, special education, rehabilitation counseling, or psychology
- One year of supervised social work experience in a health care setting
- License Social Worker License in required state
