Job Summary
Directs the Social Services department in a post-acute care facility to meet medically related social and emotional needs of patients in accordance with laws, regulations, and Life Care standards. Responsibilities include implementing a social services program, acting as patient advocate, educating staff on patient rights, charting promptly and accurately, assisting patients and families with education and financial planning, liaising with community agencies, recruiting and supervising social services staff, and maintaining professionalism, confidentiality, and compliance with company policies. Must be currently licensed/registered in the applicable state and maintain licensure throughout employment; Bachelor's degree is required for facilities with 120+ beds.
Required Qualifications
- active state license
- Bachelor’s degree if facility has 120+ beds
- Two years’ experience in health care social work preferred
