Social Services Director

The Social Service Director leads resident psychosocial services in a long-term care setting, overseeing patient-centered care through the Interdisciplinary Team (IDT) and ensuring timely MDS assessments, discharge planning, and referrals to psychosocial clinicians. Responsible for coordinating with case managers and external providers, handling PASRR reviews, maintaining HIPAA-compliant documentation, and supporting quality assurance/performance improvement (QAPI) initiatives. The role includes promoting resident rights, staff training, community outreach, and active involvement in care planning, QA meetings, and regulatory compliance. Must participate in daily operations, emergency availability, and representation of the facility to residents, families, vendors, and the broader community.
