Job Summary
Directs social services (social work) for residents in a post-acute care setting, evaluating resident needs, coordinating services with the interdisciplinary team, and overseeing case management; may supervise other staff and contribute to program administration.
Required Qualifications
- At least 1 year experience in health care setting
- Excellent written and verbal communication skills
- Computer proficiency with Microsoft products
- Post-acute care or health care experience preferred
- Bachelor’s Degree in social work (SW) or social welfare from an accredited institution preferred
- Master’s Degree preferred
- Current state license as a Social Worker preferred
