Social Services Director

Directs social services operations in a 120-bed facility, overseeing admissions/discharge processes (including referrals, screenings, tours, and financial assistance), patient assessments and care planning, supervision of social services staff, and advocacy for residents. Ensures regulatory compliance, maintains detailed documentation, participates in inspections and leadership meetings, contributes to policy development and strategic decisions, educates stakeholders on resident rights and safety, and collaborates with the interdisciplinary team to prevent or resolve escalated situations while fostering family involvement. Preferred Minnesota social work license; healthcare/long-term care experience and background check required.
