Job Summary
Manage the medically related social service program by meeting identified needs, arranging personal and adaptive services, and providing or securing counseling for residents. Track psychosocial needs, maintain a reference library of outside resources, and assist with transportation and admission scheduling while ensuring compliance with resident rights and facility policies. Participate in assessments, care planning, and discharge planning to support transitions to outside living arrangements. Administer the Advance Directive program, develop relationships with regulators and the community, and attend in-service training to prevent abuse and neglect. Maintain a complaint log and assist with Resident Council meetings. Work in a fast-paced nursing facility environment requiring prolonged standing, walking, and the ability to communicate effectively with residents, families, and healthcare professionals.
Required Qualifications
- At least 3 years of long term care experience
- Four (4) year degree in Social work or related field
- Eligible for licensure in the state in which the facility is located
- Good standing with the state
- Ability to lift
- Ability to carry
- Ability to climb
- Ability to reach – at and below shoulder height
- Ability to push/pull
- Ability to grasp and handle – pens, paperwork and small equipment
- Ability to sit, stand and walk
- Ability to twist, bend, stoop, kneel and squat
- Fine hand motor coordination
- Ability to read and write legibly
- Ability to give and follow instructions
- Ability to communicate with residents, families, physicians, visitors and staff
- Ability to communicate on the phone effectively
- Ability to take instructions from the Administrator
- Ability to respond to resident complaints and requests
- Ability to respond appropriately to disaster instructions and pages
- Ability to detect and maintain facility odors
- Ability to tolerate a high pace as typical for a nursing facility
- Ability to take and give instructions well
- Ability to enjoy working with the elderly
- Ability to sit for prolonged periods of time
- Ability to tolerate prolonged standing and walking
