Social Services Director

Coordinate all facility admissions, including re-admissions, room moves, and patient inquiries. Manage discharge planning by writing comprehensive plans that involve family and community services. Coordinate self-governing resident and family councils while addressing grievances and acting as the resident advocate when necessary. Participate in the interdisciplinary team to facilitate care planning and referrals to other healthcare providers. Requires a Bachelor's Degree in social work, current state license, and at least one year of supervised healthcare experience.
