Job Summary
Act as a liaison for residents' social interests and direct the overall operation of the Social Services department in accordance with federal, state, and local standards. Develop comprehensive care plans that identify emotional, psychological needs, strengths, and gaps while maintaining ongoing assessments. Establish and maintain relationships with residents and families regarding their rights, encouraging participation in care plan development and review. Ensure medically related emotional and social needs are met individually for long-term and short-term care populations in a geriatric rehabilitation environment. Maintain resident confidentiality and uphold a positive attitude in communication with the elderly.
Required Qualifications
- Must be at least 18-years of age
- Bachelor's degree is Social Work or related field
- Must have two years of previous experience as a Social Worker
- Must be able to communicate effectively, both verbally and in writing
- Must maintain the confidentiality of the residents, comfort and positive attitude in communicating with the elderly
Desired Qualifications
- Previous SNF or hospital experience preferred
