Social Services Director

Deliver medically related social services to attain or maintain the highest level of physical, mental, and psychosocial well-being for residents. Conduct psychosocial assessments and interventions while managing case management, discharge planning, and crisis situations. Facilitate empathetic engagement with residents and families, resolving conflicts and mediating disputes through clear interdisciplinary communication. Collaborate with departments, consultants, and government regulatory agencies to promote cross-departmental teamwork and excellent care outcomes. Maintain a comforting atmosphere that reflects C.A.R.E. values of Compassion, Accountability, Relationships, and Excellence.
