Social Services Director

Deliver medically related social services to attain or maintain the highest level of physical, mental, and psychosocial well-being for residents. Conduct psychosocial assessments and interventions while managing case management, discharge planning, and crisis situations. Facilitate empathetic engagement with residents and families, resolving conflicts and mediating disputes through clear communication with interdisciplinary teams. Collaborate with departments, professionals, consultants, and government regulatory agencies to promote cross-departmental alignment. Create a comforting atmosphere that supports geriatric advocacy and senior care excellence while upholding Compassion, Accountability, Relationships, and Excellence values.
