Job Summary
Develop social histories and comprehensive care plans for patients/residents to address medically related social and emotional needs. Design and implement group and individual social service programs while coordinating discharge planning with physicians, staff, and families. Participate in interdisciplinary conferences, complete MDS documentation, and enforce abuse prevention policies to ensure resident safety. Orient families to the facility environment and provide crisis support as needed. Serve on committees and lead in-service training for staff.
Required Qualifications
- Minimum of a Bachelors degree in social work or in human services fields.
- Licensed per the state requirements or eligible for licensure.
- Skills in communication, documentation, assessment, and social work methods and techniques.
- Has at least one year of experience in long-term care setting and/or related healthcare setting.
- Ability to read, write, and speak the English language.
- Ability to function as a Team Leader/Role Model.
- Has reviewed Cantex Continuing Care Networks Clinical Policies and Procedures or Abuse Prevention and knows the employee's responsibility to enforce it.
- Responsible for assuring patient/resident safety.
