Job Summary
Social Services Designee facilitates admission and discharge processes, markets services, builds relationships with referral sources, and clarifies legal documents for residents and families. Participates in interdisciplinary care planning, identifies psychosocial needs, assists with developing individualized plans of care, and provides medically related social services to help residents attain or maintain the highest possible functioning. Responsibilities include admission/discharge coordination, resident and family support, liaison with staff and community agencies, care planning participation (PHQ-9, BIMs, MDS), professional conduct, and compliance with regulatory requirements. Location: Bloomington, MN; on-site with full-time schedule.
Required Qualifications
- Experience in healthcare, skilled nursing facility or assisted living community
- Must recognize and understand the psychosocial and emotional needs of residents
- Strong skills in communication, organization, management, and leadership
- Must be able to perform essential functions of the position with or without reasonable accommodation
