Job Summary
Social Services Coordinator responsible for assessing residents’ psychosocial needs, completing social histories and designated MDS sections, documenting care plans and progress, and providing counseling and emotional support to residents and families. Coordinates discharge planning and care transitions, facilitates referrals and collaborates with the care team to enhance residents' quality of life and outcomes. Plans and supports resident, family, and community engagement activities and promotes facility programs to referral partners. Requires a bachelor’s degree in social work, gerontology, or related health sciences, and eligibility to obtain an LSW, plus at least one year of supervised health-care social work experience. Strong interpersonal and communication skills, empathy, patience, and discretion are essential.
Required Qualifications
- Bachelor’s degree in social work, gerontology, or a related field within health and human services including sociology, special education, rehabilitation, counseling and psychology
- Ability to obtain a Licensed Social Worker (LSW)
- Minimum of one year of supervised social work experience in a health care setting working directly with individuals
- Interpersonal and communication skills enabling positive and accurate communication
- Strong commitment to empathy, respect, patience, and discretion
