Job Summary

Evaluate resident needs for social services and coordinate care with the interdisciplinary team, family members, and community resources. Provide services per resident care plans, oversee case management, and maintain comprehensive documentation. Deliver specialized training to staff and participate in clinical conferences and performance improvement committees. Adapt policies, stay abreast of procedural changes, and make recommendations to center leaders while adhering to all applicable laws and regulations. This part-time role involves significant physical activities including lifting up to 50 pounds and assisting with resident transfers.

Required Qualifications

  • Ability to read, write, speak, and understand English
  • Meets all health requirements, as required by law
  • Must be able to lift up to 50 pounds unassisted
  • Must be able to bend, stoop, push, pull and twist
  • Must be able to provide lifting and transfer assistance to residents
  • Must be able to use a lifting device and/or assistance of other staff when required
  • Must be able to stand
  • Must be able to perform significant physical activities
  • Must be able to maintain appropriate documentation in a timely and comprehensive manner

  • Must be able to provide specialized training to other staff, family members, consultants and other individuals as needed
  • Must be able to participate in clinical conferences and in-service programs to share information with the interdisciplinary team
