Job Summary
Coordinate resident transitions across Independent Living, Assisted Living, Memory Care, and Skilled Nursing while assessing social, wellness, and support needs. Build meaningful relationships with residents and families to connect them with internal and external community resources. Monitor resident wellness, proactively identify support opportunities, and advocate for residents as a liaison to physicians and community departments. Lead interdisciplinary meetings, organize health education programs and support groups, and guide new residents through orientation. Maintain confidentiality and manage multiple priorities in a fast-paced senior living environment.
Required Qualifications
- Bachelor's degree in Social Work or a related field
- Level II background check
- Employment contingent upon successful completion of background screening
Desired Qualifications
- Experience in senior living, healthcare, case management, social services, or care coordination
- Strong interpersonal, organizational, and communication skills
- Ability to maintain confidentiality and exercise sound judgment
- Ability to manage multiple priorities in a fast-paced environment
