Job Summary
Conduct interdisciplinary assessments for adult and geriatric residents to address physical, psychological, and economic concerns. Formulate care plans, facilitate care conferences, and investigate grievances while aligning resources with age-related protocols. Communicate verbally and in writing with residents, families, and the healthcare team to establish rapport and support optimal functioning. Complete assigned assessments and utilize critical thinking for independent judgment within the senior care community. Requires CNA certification within six months of hire.
Required Qualifications
- Minimum high school graduate or equivalent
- Certified Nursing Assistant (CNA) certification
- Working knowledge of the principles of teamwork
- Knowledge regarding the impact of illness on the resident/family
- Skill proficiency in verbal and written communication
- Ability to establish rapport with residents, families and the healthcare team and community
- Critical thinking skills for independent judgement
- Ability to actively participate with the healthcare team and community agencies
Desired Qualifications
