Job Summary
Coordinate social services for residents in an 85-bed long-term care skilled nursing community. Assess residents’ psychosocial needs, complete social histories, and document care plans and progress; provide counseling and emotional support to residents and families; assist with discharge planning and care transitions; coordinate referrals and liaison with community resources; partner with the care team to enhance quality of life and outcomes; plan and support resident, family, and community engagement activities; promote facility programs and services to referral partners. Requires a bachelor’s degree in social work, gerontology, or related health and human services field and ability to obtain a Licensed Social Worker (LSW). Minimum of one year of supervised social work experience in a healthcare setting.
Required Qualifications
- Bachelor’s degree in social work, gerontology, or a related field within health and human services including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.
- Ability to obtain a Licensed Social Worker (LSW).
- Minimum of one year of supervised social work experience in a health care setting working directly with individuals.
