Job Summary
Confer with residents and family members to develop comprehensive social histories and psychosocial assessments, including cultural and religious background preferences. Orient new residents to facility services and rights during admission, while completing MDS assessment sub-sections and Pre-admission screening for Medicaid eligibility. Assist in developing, implementing, and evaluating care plans, participating in quarterly conferences with physicians and care teams. Provide support for emotional distress, actively engage in QAPI program improvement, and ensure all reports meet state and federal documentation timelines. Maintain regular attendance and complete daily assignments with compassion for the elderly population.
Required Qualifications
- Bachelor's Degree in Social Work, Human Services, or related field
- One year of supervised social work experience in a health care setting
- Iowa Board of Social Work licensed
- CPR Certification
- Resilience: ability to welcome constructive feedback, course correct and not take it personally
- Personability; ability to get along with all personality types and inspire trust with residents, staff, family
- Ability to actively listen, with the goal of understanding
- Ability to clearly speak and proficiently read, write and understand English
Desired Qualifications
- One year of supervised social work experience in a health care setting, working directly with individuals
- Iowa Board of Social Work licensed
- CPR Certification
