Social Services Assistant

Collaborate with the interdisciplinary team to promote resident rights and well-being by educating staff on social services roles. Participate in developing written interdisciplinary plans of care to address social services needs for each patient. Conduct meetings with residents, families, and significant others to provide support and improve quality of life. Perform discharge planning activities including family education, documentation, and referrals to appropriate agencies. Establish and maintain relationships with community-based agencies to ensure referral flow. This part-time role offers 20 hours per week at Westgate Hills Rehabilitation and Nursing in Havertown, PA.
